"If one feels that the hospital did not report the sentinnel event, can family contact the Joint Commission to see if it was done?"

"Thank you and most informative. Peer review is necessary. I am a family medicine doctor. My Dad's attending failed to see the decubitus ulcer on my dad's leg in the hospital which required amputation. The mistake was between him and the wound care team. Peer review led to important chances in the electronic medical record of check lists being made. The attending went through further peer review. As you know family are not privy to what happens to the physician in these meetings. Although, I wish part of this would change. My father eventually lost his life. I am thankful that changes were made in the protocols so other patients would not encounter this, but the physician continues to practice and I am unsure if there was any remediation that took place. Frustrating."
