"I  hope I am incorrect, but of the three time consuming tasks that you mention in your last sentence, I recall (hopefully incorrectly) that only the last item will count toward the 50% of face to face time, unless the other two are done in the presence of the patient.  The typical office schedule would likely make the first two tasks done either by staff or separate from the E/M time with the patient/family.
